Social Worker

As an Adult Social Worker within the South Community Team, you will manage a complex caseload of adults with care and support needs, completing assessments, reviews, and support planning in line with relevant legislation, including the Care Act 2014, Mental Capacity Act 2007.
You will work closely with individuals, families, carers, health colleagues, and partner agencies to develop personalised support plans that promote independence, wellbeing, and positive outcomes. The role includes undertaking assessing under The Care Act through the 3 Conversations Model, completing safeguarding enquiries, assessing risk, coordinating care packages, and ensuring that support remains effective through regular review.
You'll be expected to make sound professional decisions, maintain accurate records and reports, and contribute to a collaborative team environment focused on delivering high-quality social work practice which is underpinned by legislation, theory and policy.
