Full time 36 hours per week: - 9-month fixed term contract

An exciting opportunity has arisen in Adult Social Care within Sefton Council.

Job requirements:

As a Community Assessor, you will be responsible for providing practical and social support for vulnerable people and their families, including contributing to the assessment and review of care packages, recognising serious risk and promoting informed choices.

You will manage a defined case load delivering high quality assessments and support plans that address the needs of vulnerable people through the commissioning of individual support, whilst managing the reputation of the Council.
