An exciting opportunity has arisen in Adult Social Care within Sefton Council.

 

Job requirements:

As a qualified Social Worker in a busy, hospital Social Work team you will be responsible for fulfilling the Local Authorities statutory duties under the Care Act. This includes assessing individuals and arranging practical and social support for vulnerable people to facilitate a safe and timely discharge from hospital. 

You will manage a defined case load delivering high quality assessments and support plans that address the needs of vulnerable people through the commissioning of individual packages of care, whilst upper holding the excellent reputation of the Council.
