You will be responsible for managing a caseload of children and young people with SEND where they are undergoing an Education, Health and Care Plan Needs Assessment, have an Education, Health and Care Plan and support from social care, or where they are referred to the SEND Social Care Team.

You will lead on undertaking social care assessments of need as part of the Education, Health Care plan Assessment and/or review and undertake reviews of social care packages of support on an annual basis in partnership with the child/young person, their families and partner agencies such as schools and health to ensure that the support provided continues to meet need and continues to support better outcomes.

You will be responsible for ensuring that all assessments and plans progress in a timely manner and intervention is effective to support children within their families and prevent the need for the plan to escalate.

In Knowsley our SEND Social Care Teams are responsible for support and interventions for children and young people with disabilities from birth up to the age of 18 years.
