Outreach Support Worker

 

This is an exciting opportunity to be part of a new team within the Aiming High Outreach Service.

Aiming High offer short breaks to families as part of the children With Complex Needs Services in Children’s Social Care.

This is an innovative new service, launched earlier this year.

The successful applicant will have experience working with children and young people with complex needs and their families.

The Outreach Worker will have an ability to communicate effectively and ensure that the children and young people feel understood, valued and safe.  They will work intensively and holistically with a child or young person in the home and community.  They will be guided by their assessed needs.  They will support them to meet measurable outcomes within a defined timescale.  They may support their interests, skill development and their emotional and mental wellbeing.
