Make A Difference as a Team Supervisor in our Reablement Team. (In the private sector this can be called Care Co-ordinator)

We are seeking to employ a full time Reablement Team Supervisor who will enjoy working with adults aged 18+, and within a dedicated, friendly and supportive team.

The aim of the service is to support vulnerable adults to be as independent for as long as possible, safely in their own homes. You must be flexible and able to use initiative when putting into practice care support/ rehabilitation programmes set up alongside professional staff.
We are also about to start our exciting new collaboration with the Whiston Hospital Discharge team called Home First which we hope will speed up safe discharges from the Hospital.

The successful candidate will need to demonstrate their ability to support staff, have experience with electronic care systems, engage with both Health and Social Care professionals and with service users in the community, prevent hospital admission / facilitate timely hospital discharge.
Our office is Based at Nightingale House (adjacent to Whiston Hospital), our service operates from 07.30am to 10pm, 365 days per year.

You will work Monday to Friday 37 hrs.

We will provide you with the necessary training and support to enjoy and to succeed in the role.
