The Rapid Response service are looking for Rapid Response Assistants to work in a multi-disciplinary team of professionals working in delivering a 24hr response service to avoid hospital admission and to facilitate hospital discharges. You would carry out person centred care offering practical support and advice.
This is a developing service with great career prospects and training on offer, you must hold a Valid driving license and have access to a car.

The service is open Monday to Sunday 8am till 10pm 365 days including Bank Holidays.
