Joining us as an experienced Occupational Therapist means you will provide expertise commensurate with your role, experience and grade acting as invaluable resource to the team and department working in a diverse, challenging and rewarding environment.
We are seeking a well-motivated and enthusiastic HCPC registered Occupational Therapist, with good communication skills and the ability to work under pressure, to join our team. The successful applicant will have an interest in developing their skills working with adults with disabilities who have complex needs and will be responsible for the provision of specialist assessments and follow-on support to those in need of residential or nursing home placement. The successful applicant should have a good understanding of complex physical disabilities, mental health conditions in particular dementia diagnoses, and experience in the provision of equipment. The trusted assessor role involves assessing patients on wards and intermediate care settings to facilitate discharge to a care home residence, this can be new placements or returning placements. You will also support those people once discharged from hospital to ensure a smooth transition into their new home. You will play a key role in an integrated team to facilitate timely discharge from hospital with the person being at the centre of the assessment and recommendations made. Knowledge of up to date moving and handling techniques and good IT skills would be an advantage. The Service is committed to continuing professional development.
