Shared Lives Placement Officer 

About the role:

An exciting opportunity has arisen in Wirral Shared Lives scheme for Shared Lives Placement Officer. The role will be to work as part of a team supporting the Registered Manager and Senior Placement Officer to monitor and manage a case load of Carers, ensuring compliance with quality standards in-line with CQC Regulatory Framework, The Care Act 2014 and Shared Lives Policy and Procedures. 

Using your knowledge and experience in care and support planning, gained from direct work with people with support needs, you will ensure plans are in place for people supported, responsive to changing needs, promoting independence, wellbeing, and control to ensure the continuity of a high-quality service and satisfaction.

You will manage a caseload of approved Carers, carrying out regular monitoring of training and review visits to offer support and advice as appropriate and to ensure they maintain accurate records of the people supported. 

About the Candidate:

You will have a sound knowledge of person-centred practice matching individuals and the ability to support change and improvement in Shared Lives carers practice creatively and flexibly, working in partnership in a professional way to ensure successful outcomes.

You will have the ability to effectively assess, recruit and train Shared Lives carers to provide a service to people with eligible care and support needs, with a person-centred approach being the foundation for building successful arrangements, producing reports for Panel within a deadline.
You will have the skills to promote, liaise, negotiate, and communicate with a range of professionals to publicise the Shared Lives service. 

You will hold a Level 3 Diploma in Health and Social care or comparable, or a willingness to work towards it. 

You must be able to travel around the borough, proactively work to deadlines, and undertake administrative duties and use ICT systems effectively.
