We are interested in hearing from you if you work with children or young people and their families and want to join the Care Leavers team here in Halton as a Personal Advisor.

 

A permanent vacancy has arisen within the Care Leavers Service.  This is an existing opportunity to join an established group as a Personal Advisor within a busy and highly motivated team.

 

As a Personal Advisor, you will be responsible for assessing and supporting Care Leavers post 18 and supporting them pre 18 alongside their social worker. You will build working relationships with the young people in order to support them as they transition into adulthood. Within the Personal Advisor role, you will be responsible for managing your own diary around the needs of the young people you work with. You will support young people to navigate health services, education provision and housing services to name but a few. This will include chairing meetings, devising Pathway Plans, attending appointments with young people and working with partner agencies.

 

The role will involve regular travel across the borough and sometimes further afield. Therefore, the post holder must have a driving licence and access to a vehicle. Where necessary, reasonable adjustments will be made in accordance with the Equality Act.
