Service Manager - Integrated Social Care & Health

Transformation through the delivery of our Commissioning Strategies and Transformation Programme.
This position is a key role within the context of health and social care integration.
There has been recent significant development of our Sefton Partnership to deliver integration across health and social care, however, to continue this journey and improve outcomes for individuals and our community we need the right person to fill this critical role.
The post holder will lead on a range of developments at a strategic level and will work very closely with members of the Health and Well Being Board and key Council Elected Members.
The post is directly accountable to the Assistant Director, Adult Social Care and Health, however the postholder will also be part of the Adult and Children’s Social Care departmental teams and the NHS Cheshire and Mersey Side ICB Sefton Place team.
