Trusted Assessor - Registered Nurse.
Are you a solutions-focussed Nurse, with health or social care experience who is comfortable working across multi-disciplinary teams?
Do you have the drive needed to collaborate with a range of different services to successfully negotiate care placement for vulnerable adults with ill-health and long-term conditions?
Halton Borough Council is looking to recruit a Trusted Assessor to work alongside providers of 24-hour care to support new and existing placements out of hospital.
The post is new to the authority and will be based in it's Care Home Division.
It is expected that the post-holder will shape the position, working closely with care homes across the borough with social care and the local hospitals.
This is a vital systems role, aimed at impacting positively on health, social care and those who access services.
The successful candidate will be responsible for developing the ‘trust’ element of the role, ensuring that placements are timely, safe and appropriate.
